Proper mailing address format?

I ordered a product online and am paying for it by mail. On the website, the mailing address format is:



name

profession

street address

city, state

zip code



But I always learned that the proper mailing address format was:



name/business

street address

city, state, zip code



So, which way should I write it? I have to do it correctly because I absolutely CANNOT have the envelope returned to me!!!!!
