Thank to all of you.Can my identity be stolen by giving the following information?
I think the big thing to worry about would be your social security number and birth date, tied to your name. Sounds like you didn't give them that. Most of the other items appear to be things that just about anybody could obtain by typing your name into Google. Good for you to be concerned and stay on top of it.

My identity, and my son's, have been "compromised" several times over the last couple of years, by having been included in databases that were copied and either shared with an outside party, or lost. My last mortgage holder had a top financial officer that was caught selling the personal data of thousands of customers. My son's was part of a database that was emailed to the wrong recipients. There were a couple of other, similar occurrences, for me. Nothing came of any of the situations, but I stay wary.

I suggest for you to keep a close watch on your credit score, and make sure you are getting all of your mail. If you're really worried about identity theft, there are services you can sign up for to safe-guard your personal information. My bank offers one, for a cost. I don't know of any that are free. Can my identity be stolen by giving the following information?
It's possible. People can get about any information they want if they really want it. However you didn't give out bank account numbers or social security, driver's license, etc. It is more likely you will get sent a bunch of advertisements for stuff, but I would keep an eye on your personal accounts just in case. Can my identity be stolen by giving the following information?
NEVER give out personal info on the phone, mail and online unless you began the contact since its NOT secure! And don't send any personal by email since its also Not secure! No legit company asks for your personal info by email, phone, or mail. And yes your identity can be stolen by giving out any personal info! Now also you should be even more concerned if you gave out your ssn through that email which I hope that you didn't because your ssn is basically the magic key to do anything like like getting employment, insurance, credit, loans, etc in your name! Those emails that ask for your personal info are called (phishing emails) its when a fraudster sends you emails pretending to be a legit company in order to steal your personal info! Now vishing is when a fraudster calls your pretending to be like your bank and asks for your personal info to steal your information. Now I can't remember whats it called when a fraudster asks for your personal info by mail but they can even steal your personal info by mail so like I say don't give out any personal info unless you know who you are dealing with. Right now the best thing to do is to place a fraud alert on your credit reports by visiting www.annualcreditreport.com where you can see your credit reports to find out if anyone has been using your name to commit other crimes or fraud. Now the best thing to do is to place a security freeze on all your credit reports since that is much better than fraud alerts. You will need to place a security freeze on each of your credit reports visit the three nationwide credit reporting agencies at www.equifax.com, www.transunion.com, www.experian.com
